Request for Proposals: Hybrid Specialty Care Services
Cow Creek Health and Wellness Center (CCH&WC), a Tribal health organization operating under the Indian Self-Determination and Education Assistance Act (PL 93-638), is soliciting proposals from qualified organizations capable of providing hybrid specialty care services that integrate telehealth technology with onsite clinical support personnel.
The selected contractor shall provide comprehensive specialty consultation and care management services designed to improve access to specialty care for Tribal citizens and community members while supporting the delivery of high-quality, coordinated healthcare within the primary care setting.
Minimum Qualifications and Service Requirements
Proposing organizations must demonstrate the ability to provide the following:
- Specialty Care Access
- Access to board-certified or board-eligible specialty physicians capable of providing consultation, evaluation, treatment recommendations, and ongoing care management within one (1) week of referral from a CCH&WC primary care provider.
- Specialty services must be available in, at a minimum, the following disciplines:
- Rheumatology
- Endocrinology
- Neurology
- Cardiology
- Pain Management
- Dermatology
- Addiction Medicine
- Credentialing and Revenue Cycle Support
- The organization must be capable of credentialing and privileging its providers through CCH&WC in accordance with applicable Tribal, federal, and accreditation requirements.
- The organization must have the ability to support or perform billing functions for specialty services provided on behalf of CCH&WC and demonstrate experience with third-party reimbursement processes.
- Hybrid Clinical Service Model
- The proposed service model shall include real-time specialty consultations supported by onsite clinical staff.
- The technology platform and clinical workflow must enable consulting providers to utilize point-of-care diagnostic tools, including but not limited to:
- Ultrasound
- Electrocardiogram (ECG/EKG)
- High-resolution examination cameras
- Digital otoscopy and/or scoping equipment
- Onsite medical assistants or other qualified clinical personnel shall facilitate examinations and diagnostic procedures under the direction of the consulting specialist to support clinical assessment, differential diagnosis, and treatment planning.
- Clinical Documentation
- Consulting specialists shall complete and enter clinical documentation into the CCH&WC electronic health record system within seventy-two (72) hours of the patient encounter.
- Documentation shall meet all applicable standards for clinical care, billing, quality assurance, and accreditation compliance.
- Care Coordination and Interdisciplinary Collaboration
- Consulting specialists shall participate in interdisciplinary care team meetings, case conferences, quality improvement activities, and care coordination discussions as requested by CCH&WC.
- The organization must demonstrate a commitment to collaborative, patient-centered care and integration with the existing primary care and behavioral health teams.
- Criteria for Selection
- Preference may be given to organizations with demonstrated experience serving Tribal health programs, rural healthcare organizations, Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), or healthcare organizations operating under the 638 model. Respondents should describe their experience supporting underserved populations and improving access to specialty care through innovative care delivery models.
